Equipment records, not patient records

X-Ray and Digital Services, LLC, doing business as Fix My X-Ray, repairs, installs, and sells imaging equipment. The company is not a clinic and does not treat patients. The records office is for equipment location, ownership, service history, quotes, and invoices.

Please do not send patient information

Do not upload radiographs, photographs of a patient, or a form that names a person who was imaged. A serial-number label, a room photo with no patient in it, and a service report about the machine are appropriate. If a hospital or clinic believes a business associate agreement is required before any visit, ask for that agreement before sending anything that could identify a patient.

What this notice is not

This page is not a claim of HIPAA certification, and it is not a HIPAA seal. Certification language would be false. The protection in version 2 is practical: sign-in, a role for each person, no bank numbers stored on the site, and no patient-image feature at all.
